Incidents Adversely Affecting Health and Safety (MUI)
The Investigative Agent reviews all major unusual and unusual incidents reported to the agency. All MUI incidents are submitted to the Ohio Department of Developmental Disabilities Incident Tracking System. These reports are confidential and not public record. Reports of abuse and/or neglect involving children are reported to the Department of Children’s Services, and cases involving adults are reported to the local law enforcement agency.
The Investigative Agent makes recommendations ensuring the health and safety of those served by the agency. Training sessions are offered every 45 days on Incidents Adversely Affecting Health and Safety, and these sessions are free and open to the public.

Quality Assurance
Quality Assurance reviews offer peace of mind to those in our disabilities programs. These reviews look at satisfaction levels in housing, health, safety and services. The reviews provide an opportunity to express concerns on services provided, and discuss potential changes needed in any particular plan. Our Quality Assurance Specialist conducts reviews in both residential and day programming services.
Reviews provided include:
• Full Reviews conducted at least every three years, for all individuals receiving residential services
• Random Quality Assurance Reviews conducted on an “as needed” basis

Provider Compliance Reviews
Provider Compliance Reviews are conducted as dictated by the Department of Developmental Disabilities, or as a result of a complaint, a Quality Assurance Review, or a Major Unusual Incident. All independent providers and residential providers who work with individuals on IO and Level One Waivers are subject to Provider Compliance reviews. The Quality Assurance Specialist follows protocol as outlined by OD/Developmental Disabilities in conducting these reviews, and works with providers so that they meet all state mandated requirements.

Delegated Nursing
Warren County Board of Developmental Disabilities works with residential providers to ensure all health care needs are provided to the individuals served. Delegated nursing quality assurance reviews are mandated for individuals who receive delegated nursing services from a residential provider. These reviews can only be conducted by a trained R.N. An assessment is completed annually to determine the need for these delegated nursing services. These reviews must be completed by the County Board once every 3 years.
